What is a Case Manager? - A Case Manager is a person who is engaged to assess the rehabilitaion needs of an individual, coordinate and implement services and therapies and is the central point of communication for all key stakeholders.  

At Rehability our Case Managers can provide the following services to you:
·         Comprehensive rehabilitation needs assessments - identifying therapeutic needs for catastrophically injured clients

·        Coordination of rehabilitation programs and services

·         Liaison with funding providers

·         Referral to relevant agencies, if required

·         Coordination of support workers, if required

·         Knowledge of rehabilitation and insurance systems

·         Community re-integration and transition planning
